New Program Site

We’re thrilled to announce the opening of an additional Secondary Transition site in the fall of 2025. This new site will align with our mission to help students build essential life skills, gain meaningful hands-on experiences, and grow in independence. The program will provide important linkages and supports to bridge the transition from school to adult life. Like our other Secondary Transition sites, it will focus on daily living skills, supported work experiences, healthy relationships, recreation, and community inclusion.

Program Description

This program provides a structured, supportive environment where students can learn practical skills through a combination of classroom instruction and real-world experience. With personalized guidance from teachers and job coaches, students gain the confidence and competence to succeed across multiple settings. 

Key Areas of Focus: 

Daily Living Skills: 

  • Nutrition 

  • Understanding and using money 

  • Functional communication 

  • Making safe choices 

  • Functional daily living skills 

  • Healthy relationships 

  • Time management (telling time and following schedules) 

  • Classroom and community safety skills 

  • Mobility (para-transit) 

  • Social skills, including conversation, self-advocacy, and requesting help or breaks 

Work Experience and Volunteering: 

  • Pre-vocational training 

  • Supported internship opportunities 

  • Job readiness skills, including communication, task completion, and teamwork 

  • Safety while engaging in work tasks 

Community Inclusion: 

  • Encouraging positive recreational experiences 

  • Socializing and engaging in community activities within the educational setting 

  • Connecting to outside agencies to bridge school to adult life 

  • Safety in the community
